Who cryolipolysis does not suit

The honest limits of fat freezing.

It reduces defined pockets. It is not a weight-loss treatment, and for a meaningful number of people who ask for it, it is the wrong tool.

We turn people away from this treatment more often than from anything else we offer. Not because it does not work — it does, for the thing it is designed for — but because the thing it is designed for is narrower than the marketing around it suggests. This guide is written to save you a consultation if the answer is already no.

The short answer

Cryolipolysis suits someone close to their stable weight with one or two defined, pinchable pockets that have not responded to diet or training. It does not suit someone looking to lose weight, someone whose main issue is loose skin rather than fat, or someone who wants a general reduction across a whole region. If you cannot pinch it between your fingers, the applicator cannot hold it, and the treatment cannot work on it.

How it actually works

An applicator draws tissue in and holds it at a controlled low temperature for the length of the cycle. Fat cells are more vulnerable to cold than the surrounding tissue, so a proportion of them are damaged and cleared by the body over the following weeks. Results build over eight to twelve weeks — there is nothing to see the same day, and anyone promising otherwise is describing a different treatment.

That mechanism explains every limit below. The applicator has to grip the tissue. The tissue has to be fat rather than skin. And the body has to do the clearing, which is why the result arrives slowly and why overall health and weight stability matter to the outcome.

We will say no if

Weight loss is the goal. This is a contouring treatment, not a weight-loss one, and it will not change the number on the scale meaningfully.

The tissue cannot be pinched. Firm, non-pinchable abdominal fullness is often visceral, sitting beneath the muscle wall where no applicator can reach it.

Loose skin is the real complaint. Removing fat beneath slack skin generally makes the slackness more visible, not less.

Your weight has moved significantly in the last six months, in either direction. We want a stable baseline before treating.

You have a hernia at or near the treatment site.

You have cryoglobulinaemia, cold agglutinin disease, or paroxysmal cold haemoglobinuria — cold-related conditions that make this unsafe.

You are pregnant or breastfeeding.

There is active infection, an open wound, or a recent surgical scar in the treatment area.

You need a result in under two months. The timeline is what it is.

What we recommend instead

If skin laxity is the primary issue, EvolveX or Morpheus8 address firmness rather than volume, and a course of either will do more for how the area reads than removing fat from underneath it would. If the presentation is heaviness, congestion and dimpling rather than a discrete pocket, Endosphères is the more appropriate treatment. And if the honest answer is that a stable weight has not been reached yet, we will say that too, and suggest you come back afterwards. Treating a pocket that is about to change is spending money on a moving target.

Paradoxical adipose hyperplasia

A rare but documented complication in which treated tissue enlarges rather than reduces, appearing some months after treatment. It is uncommon, it is more frequently reported in men and on the abdomen, and it does not resolve on its own — surgical correction is usually required. Any practice offering this treatment should raise it with you unprompted, before you book. We do, and we put it in the written plan.

If you are a good candidate

Expect roughly two cycles per area, and a result that builds over eight to twelve weeks.

Expect the area to be numb, tender and sometimes bruised for a week or two afterwards.

Expect us to map and photograph applicator placement so each session repeats exactly.

Expect a written plan with the number of cycles and the cost before you commit to any of it.

Expect us to review at twelve weeks and tell you honestly whether a second cycle is worth it.

The treated cells are cleared permanently, but the remaining cells can still enlarge with weight gain. The result holds at a stable weight and does not survive significant gain.

We describe this as a change in contour rather than a number, because the honest answer varies by area, tissue and individual. Anyone quoting you a precise percentage online is guessing.

Intense cold and strong suction for the first few minutes, then numbness for the rest of the cycle. The tender, tingling recovery over the following week is usually more noticeable than the treatment itself.
